RELEASE AND AFFIDAVIT

I hereby give The Elevation Group, Inc, its representatives and assigns, employees, or any person, persons, corporation or corporations acting under its permission or authority, or any person, persons, corporation or corporations for whom it might be acting, the right and permission to publish, reproduce, distribute and/or otherwise use any still or moving photographic or sound recording of me in whole or in part including but not limited to any statement or endorsement (including any letter, email or photograph) (the “Performance”) or any portions thereof (the “Testimonial”) regarding or related to the products and services offered by The Elevation Group, Inc or any derivatives thereof (the “Product”), in such manner, for such purposes and with such frequency as it shall determine in its sole discretion without further compensation or consideration to me and without further authorization by me.
 

(1) This is to confirm that I personally provided the testimonial information, I have reviewed the following testimonial, which I have given to The Elevation Group, Inc including my release and consent to use. The information contained in the testimonial is based on my personal information. This unsworn letter verifies under penalty of perjury the following information:

- is not misleading;

- is accurate and based on my actual experience; and

- does not misrepresent any facts relating to the use of the The Elevation Group, Inc’s products and services.
 

(2) I acknowledge that I have not received any financial incentive to give this testimonial or to allow The Elevation Group, Inc to use my story in its presentation (although I may have received a nominal reimbursement for my time and expenses); and
 

(3) That there exists no material connection between myself and The Elevation Group, Inc.
 

I hereby waive all rights of inspection or approval with regard to any recording, taping, reproduction, proposed printed, audio or video publication and/or other use of the Performance and/or Testimonial. I understand and agree that when my testimonial is publicly used, The Elevation Group, Inc will only reference my first name and last initial, and will not use my full name. By providing this testimonial and affidavit to The Elevation Group, Inc, I affirm that I am over eighteen years of age and competent to contract in my own name. I have read this release and affidavit and warrant that I fully understand the contents thereof. My provision of this testimonial and affidavit constitutes my agreement to all terms stated herein without the requirement for a written signature.
